Claude Code tightens permission checks during plugin restarts

Anthropic released Claude Code 2.1.292 on October 6 with fixes for permission checks during plugin restarts and the handling of CLAUDE.md instructions in code review.

Permissions have to survive a restart

The release notes say tool calls could be answered without plugins’ permission hooks while the hooks worker restarted. Another fix prevents a tampered cache of managed settings from disabling the built in policy plugin when a settings fetch fails.

Anthropic also reports a repair for approval bypasses on reads from network paths. Its permissions guide explains why a file read can warrant care on Windows. Accessing a network share can transmit Windows credentials to the named host.

The same guide separates tool permissions from operating system sandbox restrictions. A rule affecting a named file tool does not cover every indirect file access by another process. Teams need to verify which controls their actual workflow uses.

Keep review rules anchored

Code Review now uses the base branch version of CLAUDE.md when the pull request edits that file. This addresses reviews ignoring those instructions, according to the release.

Anthropic’s review documentation says CLAUDE.md supplies project context and new violations normally appear as minor findings. REVIEW.md sets instructions specific to reviewing, including severity. Code Review posts findings and leaves approval and merge decisions with the existing workflow.

A practical upgrade check is a disposable pull request that changes a project rule alongside code covered by it. Check which rule the reviewer applies and inspect the findings. Separately test a denied action after a plugin restart before relying on that policy for routine work.
